How to refer your patients to us

You will find below the documents you need to send us, depending on the service you require.

Programme Rachis

Interdisciplinary rehabilitation programme dedicated to back issues.

Documents to send:

Physiotherapy prescription (9 sessions)

Occupational therapy prescription (9 sessions)

Assessment by an occupational therapist

Determine the impact of the patient’s symptoms on their daily occupations and whether they would benefit from functional rehabilitation. A report is sent to you with our treatment recommendations.

Document to send:

Occupational therapy prescription (2 sessions)

Evaluation of Occupational Performance (EPO)

Measure the patient’s physical abilities through a series of functional tests representative of the demands of daily and/or professional life.

Documents to send:

EPO form

Occupational therapy prescription (9 sessions)

If an interpretation is needed: complete medical file

Pre- and post-operative programme (P.O.P.O.)

Pre- and post-operative rehabilitation programme.
